Nigel Brooks, a telehealth software developer and cloud engineer based in Houston TX, built a custom patient acquisition system for a telehealth practice. The requirement was clear: the moment a new patient completes payment, they are automatically onboarded into the practice’s clinical system. No staff involvement. No manual steps. No delay.

The Challenge

A custom acquisition layer that feeds directly into the clinical backend.

The practice had the clinical infrastructure in place: licensed providers, compliance, and patient records. What they needed was a purpose-built digital acquisition channel that could convert new patients online and route them into their backend system without any manual handoff.

The core requirement: the clinical system was the system of record. Any patient who signed up needed to exist in that system with the correct plan, membership tier, and access level before they could receive care. The connection between the patient-facing site and the clinical backend had to be real-time, reliable, and completely invisible to the end user.

The Build

Three systems. One seamless handoff.

The patient-facing site was built as a fully custom web application branded entirely to the practice. It presents membership plans, handles marketing, and owns the complete pre-purchase experience from first visit through checkout.

Behind checkout sits a payment processor integration built to do more than collect money. The moment a transaction is confirmed, the processor fires a webhook to a server-side handler that extracts the patient’s details including plan type, contact information, and membership tier, then formats them for the clinical system’s API.

That call hits the practice’s backend directly, creating the patient record in real time. By the time the confirmation screen loads, the patient already exists in the clinical system with the correct plan and access level. The practice’s staff never touches it.
